Personal Account Information
These fields appear in the Personal section of your user
profile. All fields in this section are free text entry, unless noted.
Note:
A red asterisk (*) indicates a required field.
You can also upload
a picture for your user account. To upload an image, click 
, and select a file from your local file browser.

| Middle Initial | Enter your middle initial. Note: You can only enter one character
(letter, number, and so on). |

| Legal Last Name | Enter your legal last name. |
| Nickname | Enter your nickname |
